How to Enroll

There are two ways to enroll in US Family Health Plan. You can enroll in US Family Health Plan by either downloading the paper application or through the TRICARE Beneficiary Web Enrollment Website-TRICARE Online portal.

To Download the Paper Application

To Download the TRICARE Young Adult Program Application

• TRICARE Young Adult Program Application Form

To Download the Allotment Authorization Form

• Enrolled in TRICARE Prime prior to October 1, 2011
• Enrolled in TRICARE Prime on or after October 1, 2011

PLEASE NOTE - The application allows you to type directly onto the form before printing. You cannot save data typed into this form. Print an additional copy for your records.

Print and sign your completed application. Mail it to:

To log on to the BWE web site, one of the following is required:

• Valid Certified Common Access Card (CAC): For information on how to obtain a CAC or reset a CAC PIN number, visit the Rapids Site Locator, or call the Defense Manpower Data Center Support Office toll-free at 1-800-477-8227 for assistance in locating a CAC card-issuing facility.
• Defense Financial and Accounting Services (DFAS) myPay PIN: If you have lost, do not remember, or have not received a DFAS PIN, visit the MyPay web site. Select the “New PIN” button, and follow the instructions; or call the Customer Support Unit toll-free at 1-888-DFAS411 (1-888-332-7411).
• Family Member Account: As a sponsor, you may obtain a DoD Self-Service Logon for family members by using your CAC to log on to the DoD Self-Service Access Center.
