With USFHP, you receive a fully covered annual eye exam through your Pacific Medical Centers provider. Plus, through the More Benefits Discount Program's partnership with EyeMed, you'll enjoy up to 35% off the retail price for a complete pair of eyeglass frames and 15% off the retail price for conventional contact lenses.

Please note that the eye exam is a covered benefit only when obtained at your Pacific Medical Centers clinic. If your eye exam is provided by an EyeMed provider, you will receive a discount on the cost of the exam.

Once enrolled in US Family Health Plan, you will receive a member ID card and provider directory from EyeMed. After you have your fully covered eye exam at Pacific Medical Centers, schedule an appointment at the Pacific Medical Centers Optical Shop by calling (206) 505-1200 or visit an EyeMed vision provider near you. Present your EyeMed member ID card at the vision provider's office, along with your eyewear prescription. Your vision provider will take care of the rest
